VITAIV is subject to the all privacy laws relating to medical records and also to federal and state/territory privacy laws that relate to incorporated companies. As part of the service offered and provided to you by VITAIV, it obtains and holds medical and other personal information provided by you. VITAIV only collects and holds such information as is required by law and as VITAIV needs to collect and hold in order to allow it to properly and safely provide the services you have requested. Other than as mentioned above; VITAIV will not pass on any information it has about you to any third party except to the extent that this is necessary for VITAIV to safely and properly provide the services you have requested from it. This may include other health providers involved in any treatment you receive and, only to any extent necessary, VITAIV legal and financial advisors. VITAIV may be obliged to provide in formation it holds on you to government and law enforcement agencies but will only do so to the extent that is legally required. You may request a copy of information held by VITAIV about you and VITAIV will provide such a copy within 14 days. You may be requested to contribute to the cost to VITAIV of preparing any such copy. Any such request must be in writing. You may request that VITAIV make corrections to the information it holds about you if you become aware that such information is inaccurate or wrong. Any such request must be in writing You may request that VITAIV pass on any information it holds about you to a third party. Any such request must be in writing. If you cease to be a client of VITAIV, we will only retain such records it holds about you as are required by law governing the provision of medical services and by any applicable corporations and tax laws. All directors, shareholders and employees of VITAIV are bound by this privacy policy.
